Evaluation:

Each participant has a pre and post survey for each interactive training that helps give consistent evaluation, and which is used to help adjust a personalized program plan. We have materials that the Caregivers will be required to check off with the participants, and this gives us a second point of constant evaluation. There is also a weekly/monthly report that our coordinators, teachers and mentors will be required to fill out, which will assist us in recording the consistently of progress, along with the care giver. In addition, there is a final evaluation of both the participant and the care giver, at the end of the program.

Within our work, we seek to evaluate program outcomes and plan steps to expand or improve each program and its services, as needed, in order to ensure the next group benefits from its predecessor.

Forms and methods of data collection will receive monthly review to assure that information collected is compatible with format for required reporting. Services and outcomes will be documented on appropriate forms and consistent with data collected at each program. Data also will be collected from program partners and organizations that act as referral resources for individuals and families in Hampton Roads.

Progress toward achieving the goals of separate service will be collected by the program coordinators on a monthly basis, and then reviewed with the administrator. Quarterly progress reports will assure that the Director monitors the accomplishment of each of the program goals. Their data will be used, as necessary, to determine the progress of referred participants and their reliability as a referral service provider.

These reports are will be in cooperation with the point evaluation that NSU is partnering with, that will continue to long term to adjust our services to the regions youth, and ensure we provide not only quantitative outcomes that measure numbers of services and participants, but also qualitative outcomes that assure services meet the holistic, long-term needs of participants.

Ongoing steps will be taken to improve and enhance both program service delivery and outcomes. Changes or additions to the program will be made where indicated by these reports.
